If you would like the Minister or other support person to know your wishes if you should become ill or incapacitated, you can complete the Personal Care and Support Record that will be kept on file at the church.
This PDF can be filled out digitally, saved to your computer, and then emailed to Office@ouuc.org. Alternatively, it can be printed (or you can ask for a printed copy from the office), filled out by hand, and given to Rosie in the office. Either way, the document will be attached to your Realm profile for future reference.
